Further to outside backs being held on the bench, I think a lot of it has to do with the way that the game is evolving. Once upon a time teams would carry a bench hooker as cover. With the implementation of less interchanges came the need for bigger guys to do more minutes. Backs were always doing 80.
The problem arises in that if you loose an outside back and have to shift a second rower to that position, say CTR, they can do it but a decent opposition would eat you up in that spot. If you have the luxury of bringing on a specialist that can cover one-five then beauty!
If you get an injury in the forward pack, (be it FRF, 2RF or HKR) then your big guys just have to play a few more minutes. If it's at hooker then you rely on a guy from your second row to cover the spot. Most teams have a backup option (sometimes of dubious quality) in their second row that they can rely on to play rake.
Not saying that it's a perfect theory, I just feel it's the lesser of two evils.
